Professional DiveMaster Certification Course Purchase

Regardless of your answers to the medical questionnaire, you will be required to get a doctor to sign off on your medical form as this is a professional rated certification. 

I understand this course is a Professional Rating, and will need to be available to attend class sessions to complete necessary skills to achieve.

Minimum of 48 hours notice is required on a reschedule or no show/late fees may apply. Fee for no show or last moment cancellation is $50 per instance. 

I understand that my registration will be verified and approved prior to scheduling. If not all of your certification prerequisites can be verified, this will need to be resolved prior to completing your course.
